  • Thank you for your interest in serving as a volunteer with Mt. Diablo Unified School District. This Level Three application should take about 15-20 minutes to complete.

    You may save your progress and continue at another time if you are not able to complete it entirely.

    Level Three clearance is district-wide. Once approved, you may serve as a volunteer driver for school related activities MDUSD.

    To be “cleared” as a Level Three volunteer, you must:

    • Provide a copy of your valid California driver’s license
    • Submit current vehicle information (make, model, year, license plate)
    • Upload proof of required automobile insurance coverage that meets District requirements
    • Receive final authorization and approval from the District

    You will be contacted via email once your Level Three clearance has been approved.

  • Use of Privately-Owned Vehicle for Student Transportation
    By applying as a Level Three volunteer, you acknowledge that you may be using your privately-owned automobile to transport students for school-related activities. You certify that you:

    • Have not been convicted of reckless driving or driving under the influence of drugs or alcohol within the past five years
    • Are at least 23 years of age and hold a valid California driver’s license
    • Are the owner of the vehicle being used for transportation
    • Maintain current registration and automobile liability insurance that meets District requirements
    • Agree that your vehicle is in safe operating condition. I have checked that the tires, brakes, lights, horn, and suspension, etc. are working properly
  • 🚫 Stop – Ineligible to Continue

    Unfortunately, drivers who have been convicted of reckless driving or driving under the influence of drugs or alcohol within the past five years are not eligible to apply for Level Three Volunteer Driver clearance.

    You must be at least 23 years of age and hold a valid California driver’s license to serve as a Level Three Volunteer (Authorized to Transport Students).

    You must certify that you are the legal owner of the privately owned vehicle you intend to use and that it will only be used for District-approved, school-related activities (e.g., field trips, athletic events) to serve as a Level Three Volunteer (Authorized to Transport Students).

    You must certify that your vehicle is currently registered and in safe operating condition in order to serve as a Level Three Volunteer (Authorized to Transport Students).

  • Transportation Safety Requirements
    Each passenger, including the driver, must have their own functional seatbelt. In addition:

    • Seatbelts must be worn at all times while the vehicle is in operation.
    • Children under 8 must be secured in a federally approved car seat or booster seat in the back seat.
    • Children under 12 may not ride in the front seat of a vehicle equipped with an active passenger airbag.
    • No more than seven student passengers may be transported in a single vehicle.
    • Carry only the number of passengers for which your vehicle was designed.
    • If driving a pickup truck, passengers will only be carried in the passenger compartment.
    • Student drivers are not permitted to transport other students.

  • Vehicle Insurance Requirements

    As a Level Three volunteer driver, you must certify that your automobile liability insurance meets the minimum coverage requirements as outlined below:

    • Bodily Injury Liability (BI): Each Individual – $100,000; Total Each Accident – $300,000
    • Property Damage Liability (PD): Total Each Accident – $25,000
    • Medical Payments: Each Individual – $5,000
    • Uninsured Motorist Coverage: Each Individual – $30,000; Total Each Accident – $60,000

    • Combined Single Limit (BI & PD): $300,000; Medical Payments: Each Individual – $5,000
    • Uninsured Motorist Coverage: Each Individual – $30,000; Total Each Accident – $60,000

  • Insurance & Liability Notice

    The Mt. Diablo Unified School District does NOT provide automobile insurance or medical coverage for volunteers or privately owned vehicles used to transport students. When you drive your own vehicle, your personal auto insurance is primary for any claim or loss. Volunteers transporting students in their own cars should be aware that their personal automobile insurance has sole responsibility in the case of any accident. School/district insurance does not cover the use of private vehicles; therefore, it is required that volunteer drivers have adequate insurance before transporting any student.

    Please note: Insurance policies often expire during the school year. Level Three volunteers should be prepared to submit updated insurance policy documentation upon expiration of the policy currently on file at the school site.

  • Please note that only volunteers who complete Level Two and Three clearance will be authorized to transport students.

    You will be emailed the Level Two form shortly after completing the Level Three form.

  • Driver Conduct Instructions

    As a volunteer driver, you agree to follow all applicable laws and District requirements while transporting students, including:

    • Not smoking any tobacco products (pipe, cigar, or cigarette) while transporting students, as required by law
      Note: SB 7 (Ch. 425, Statutes of 2007) added Health and Safety Code 118947–118949, which makes it unlawful for a person to smoke in a motor vehicle in which there is a minor. See accompanying administrative regulation
    • Obey all traffic laws
    • Take the most direct route to the destination or event without unnecessary stops

    In case of emergency, keep all students together and call 911 and the district office.

  • Indemnity Provision & Acknowledgment

    As the vehicle owner, you agree and accept your obligation to manage and control your vehicle in a safe and lawful manner. You further agree to defend and indemnify the Mt. Diablo Unified School District, its employees, officers, and agents from any claim, action, or lawsuit brought by anyone that arises out of, or is in any way connected to, the operation of your privately owned vehicle under this authorization.

    By signing below, you acknowledge and accept this Indemnity Provision, and confirm that you have reviewed, understand, and agree to comply with all requirements outlined in this Level Three Volunteer Application and the MDUSD Volunteer Handbook.
