Use of Privately-Owned Vehicle for Student Transportation By applying as a Level Three volunteer, you acknowledge that you may be using your privately-owned automobile to transport students for school-related activities. You certify that you:

Unfortunately, drivers who have been convicted of reckless driving or driving under the influence of drugs or alcohol within the past five years are not eligible to apply for Level Three Volunteer Driver clearance.

Vehicle Insurance Requirements
As a Level Three volunteer driver, you must certify that your automobile liability insurance meets the minimum coverage requirements as outlined below:

The Mt. Diablo Unified School District does NOT provide automobile insurance or medical coverage for volunteers or privately owned vehicles used to transport students. When you drive your own vehicle, your personal auto insurance is primary for any claim or loss. Volunteers transporting students in their own cars should be aware that their personal automobile insurance has sole responsibility in the case of any accident. School/district insurance does not cover the use of private vehicles; therefore, it is required that volunteer drivers have adequate insurance before transporting any student.
Please note: Insurance policies often expire during the school year. Level Three volunteers should be prepared to submit updated insurance policy documentation upon expiration of the policy currently on file at the school site.
Level Two and Livescan (fingerprint) Requirements
Please note that only volunteers who complete Level Two and Three clearance will be authorized to transport students.
You will be emailed the Level Two form shortly after completing the Level Three form.
The Live Scan service provider will generate your ATI number and enter it at the bottom of the Live Scan Form.
Only volunteers who complete and clear the Live Scan (fingerprint) process will be authorized to serve in a Level Three role. Instructions for completing the Live Scan will be emailed to you after submitting the Level Three Volunteer Application.

As a volunteer driver, you agree to follow all applicable laws and District requirements while transporting students, including:

As the vehicle owner, you agree and accept your obligation to manage and control your vehicle in a safe and lawful manner. You further agree to defend and indemnify the Mt. Diablo Unified School District, its employees, officers, and agents from any claim, action, or lawsuit brought by anyone that arises out of, or is in any way connected to, the operation of your privately owned vehicle under this authorization.
